All dogs residing within the Village of Hartland must be properly licensed. Please submit the following form. Once completed form, fees, and supporting documentation have been reviewed, your dog tags will be mailed to you.

Proof of Valid Rabies Vaccination: State Law requires that your dog has a current rabies vaccination.
Administrative staff will need to see proof of the rabies vaccine, such as a Rabies Certificate, showing
the producer, expiration date, and serial/lot number from the vet. Staff cannot use a rabies tag as
proof of vaccination.

Fee: Spayed or neutered pets are $15.00 per dog; intact animals are $20.00 per dog. A $5 late fee will
be added to renewal applications received after March 31, 2026.

Replacement tags are available at the Village Hall for $2.00.
